To begin a consultation with the Doctor, please complete the information below.  All information provided will be held in the strictest confidence and in full compliance with HIPAA regulations. Please note that all fields marked with an asterisk (*) are required fields to assist us in providing you with the best possible service.

Fees: The initial process, involves gathering information about you and your illness, and questions that you have. If we decide to work together, further information will be required and fees will then be discussed in detail. Most health insurance providers do not cover telephone consultations so you will be personally responsible for costs. The usual rate is $300/hour. If you have a verifiable and documented financial need, adjustments in this fee can be made.
